Job summary

The Salvation Army in New York seeks a Case Manager to deliver intensive case management for homeless families in the Shelter. You will assess clients, develop collaborative plans, and support self-sufficiency and housing placement while keeping detailed notes in the client database.

You will engage clients to reach employment and housing outcomes, coordinate with social service agencies, and participate in weekly staff meetings and supervision.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related field.
• Experience with clients pursuing independence and self-sufficiency outcomes preferred.
• Bilingual Spanish required.

Responsibilities

• Provide intensive case management to homeless families in the Shelter.
• Maintain detailed case progress notes and enter data into the approved database.
• Coordinate referrals and interfaces with social service agencies for client advocacy.
• Conduct intake and assessments, up to 24 hours after admission, and support discharge planning.
• Engage clients in housing searches and outcomes, and participate in weekly team meetings and supervision.
