The Hispanic Alliance for Career Enhancement is looking for a Case Management Coordinator to join its team in Tampa, FL. This full-time position requires the coordinator to manage complex cases, ensuring members receive comprehensive care and support.

Qualifications

• 1+ years of experience in behavioral health or a long‑term care setting.
• Ability to multitask, prioritize and effectively adapt to a fast‑paced changing environment.
• Strongly prefers Field Case Management and discharge planning experience.

Responsibilities

• Telephonically and/or face-to-face assessing, planning, implementing, and coordinating case management activities.
• Manage a caseload that includes supportive and medically complex members.
• Determine appropriate services and supports due to member's health needs.

Skills

Bi‑lingual (English and Spanish)

Computer proficiency in Microsoft Word, Excel, and Outlook

Effective communication skills

Education

Bachelor's Degree in Social Services or related field
