Job summary

CVS Health is seeking a self-motivated Case Management Coordinator in Florida to assess and coordinate member care, including face-to-face visits and telephonic support. You will manage a diverse caseload, coordinate with PCPs and specialists, and review medications to promote member wellness.

Qualifications

• Bilingual in English and Spanish.
• Must reside in Cutler Bay, FL (Zip codes 33177, 33189, 33187, 33157, 33176, 33186).
• Willing and able to travel 50-75% of time for face-to-face visits in Miami Dade.
• 1+ year of experience in behavioral health or long-term care setting.
• Bachelor's degree required, preferably in social work or related field.

Responsibilities

• Assess, plan, implement and coordinate all case management activities with members to evaluate medical needs.
• Manage a caseload including supportive and medically complex members.
• Determine appropriate services and supports: Prior Authorizations, PCP coordination, medication review, community resources.
