Job summary

Horizon Services, Inc. in Albuquerque, NM is seeking a Case Manager for Gateway Medical Sobering Center to oversee care coordination during stabilization and discharge planning for individuals with acute intoxication.

You will arrange referrals to detox, outpatient care, housing resources, and MAT, working with a multidisciplinary team. The role requires a current SUD peer support certification or NM state SUD certification, CPR/First Aid, NM driver's license, and bilingual capability.

Qualifications

• Certified Peer Support Specialists with skills and experience navigating the system of care.
• Current certification with a Substance Use state certifying body in New Mexico, or SUD Peer Support Certification.
• CPR/First Aid/ AED current certification in New Mexico.

Responsibilities

• Staff during day shift for peak discharge.
• Coordinate care and referrals for discharge planning based on ASAM criteria.
• Link clients to services and resources on the day shift.
• Coordinate transportation and warm transfers for participants.
• Provide referrals if services are not accessible on the same day.
• Coordinate the administration of the VI-SPDAT assessment and link housing resources.
