Job summary

The Association for Individual Development (AID) in Aurora, IL is seeking a Case Manager Mental Health Professional - Bilingual to join our community-based team. You will develop and implement individualized treatment plans and coordinate services to support clients’ independent living and well-being.

Qualifications include a Bachelor’s degree in human services or related field, and bilingual (English/Spanish) abilities.

Qualifications

• Bachelor’s degree in human services or related field required.
• Bilingual experience required (English/Spanish preferred).
• Some experience working in social services helpful.

Responsibilities

• Develop and implement individual treatment plans to maximize clients’ independent living.
• Complete documentation of mental health services in accordance with state standards.
• Coordinate services with community partners to meet client needs.
• Meet with assigned clients regularly and maintain daily activity logs.
• Provide client training and assist with activities of daily living as needed.
