- Minimum 1 year of experience in behavioral health or long-term care.
- Proficient with Microsoft Office and general computer use.
Responsibilities
- Coordinate care, provide education, and facilitate access to healthcare services.
- Assess member needs, identify risks, and develop and implement care plans.
- Collaborate with internal teams, providers, and external resources to address barriers and ensure benefit alignment.
- Monitor care plan progress and document activities in compliance with regulatory standards.
