Job summary

The Oregon Department of Human Services is seeking a Human Services Case Manager. The role focuses on matching client needs with available federal, state and local programs and resources through careful case management and eligibility assessment.

Qualifications

• A Bachelor's degree in Behavioral Science, Social Science, or a closely related field.
• A Bachelor's degree in any field and one year of human services related experience.
• An Associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience.
• Three years of human services related experience.

Responsibilities

• Match the needs of Aging and People with Disabilities with current federal, state and local programs or resources.
• Perform case management and screen applicants for long-term community-based care and nursing homes.
• Assess eligibility, redetermine needs, and adjust benefits and services accordingly.
• Advocate for clients and Aging and People with Disabilities within the community.

Education

Bachelor's degree in Behavioral Science or Social Science

Bachelor's degree in any field + 1 year of human services related experience

Associate's degree in Behavioral/Social Science + 2 years of related experience

Three years of human services related experience
