Qualifications

  • Three years of human services related experience.

Responsibilities

  • Match the needs of Aging and People with Disabilities with current federal, state and local programs or resources.
  • Perform case management and screen applicants for long-term community-based care and nursing homes.
  • Assess eligibility, redetermine needs, and adjust benefits and services accordingly.

Summary Of Duties

This Human Services Case Manager position will match the needs of Aging and People with Disabilities in the community with current federal, state and local programs or other resources that are available to meet those needs. This is done through case management and by screening applicants for long term community-based care and nursing homes. This position will also assess eligibility, redetermine needs, and adjust benefits and services accordingly. Also, this individual will act as an advocate for clients and Aging and People with Disabilities within the community.

Minimum Qualifications
  • A Bachelor's degree in a Behavioral Science, Social Science, or a closely related field;
  • A Bachelor's degree in any field and one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);
  • An Associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);
  • Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
Essential Attributes
  • Experience with managing services for individuals
  • Experience interpreting and implementing policy and procedures
  • Experience preparing and maintaining written narrative documentation of actions taken
  • Experience interviewing to obtain information to evaluate the next steps to take in a process
Working Conditions

General office environment. You will visit clients in their homes, hospitals, nursing homes, APD offices and other providers. A current driver’s license is required along with an acceptable driving record.
