Job summary

Growing Homecare company seeks a Case Coordinator fluent in English and Spanish to support patient care coordination across service areas. You will schedule appointments for Community Health Workers (CHW), update the master list, and maintain accurate case notes to ensure timely visits.

You will also manage client accounts, assign members, and monitor CHW performance to ensure visits are completed as scheduled, while complying with applicable policies and quality standards.

Qualifications

• Bi-Lingual English/Spanish required.
• One year homecare experience preferred.
• Experience coordinating schedules a plus.

Responsibilities
