Job summary

Help at Home in Miami seeks a Care Coordinator fluent in English and Spanish to support clients and caregivers. The role focuses on scheduling, care coordination, and delivering exceptional client experiences from the Miami office.

Responsibilities include maintaining records, HIPAA compliance, and communicating schedules with stakeholders. The candidate should be organized, compassionate, and capable of handling confidential information in a fast-paced home health environment.

Qualifications

• Bilingual in English and Spanish required; ability to communicate with clients and caregivers.
• Strong organizational skills and multitasking abilities.
• Ability to supervise staff and provide feedback to improve services.
• Proficiency with basic computer literacy and typing skills.
• Experience in health care, preferably home health; supervisory experience preferred.

Responsibilities

• Maintain client and employee files and related paperwork.
• Ensure confidentiality of patient/client information per HIPAA.
• Assign homecare aides based on client needs and Plan of Care.
• Monitor performance of aides to ensure quality service.
• Communicate schedules and changes with clients, aides, referrals, and managers.
• May conduct monthly in-home visits to connect with clients and caregivers.
• Prepare and submit routine departmental reports.
• Document client condition and service details accurately.
• Handle client satisfaction surveys, in-home visits, and employee trainings.
• Address client complaints per policy and report significant issues.
• Provide education on changes to Plans of Care.
• Maintain positive relationships with clients, aides, sources, and managers.
• Ensure compliance with laws and company policies.
• Perform other duties as assigned.
