Job summary

Jobtailor is seeking a dedicated Care Coordinator to support Health Homes program members in Everett, WA. You will manage a caseload, conduct visits, review eligibility, enroll and discharge as needed, and coordinate with healthcare teams to ensure seamless care.

This role requires bilingual communication, a Bachelor’s in a related field, and at least one year in healthcare or social work. You’ll document in the EHR, advocate for clients, and connect them with community resources while upholding

Qualifications

  • Bachelor’s degree in a related field is required.
  • Minimum 1+ year of experience in healthcare, social work, or care coordination.
  • Fluent in English, written and verbal; bilingual abilities preferred.

Responsibilities

  • Maintain a caseload of up to 50 members and conduct monthly in-person visits.
  • Review and interview potential members to verify eligibility for enrollment in the Health Homes Program.
  • Complete referrals and follow up to ensure continuity of care.
  • Orient and educate members and families about the Care Coordinator role, treatments, procedures, medications, and continuing care.
  • Initiate and reassess care plans in coordination with healthcare providers.
  • Document care and member encounters accurately and timely in the EHR system.
  • Check in with assigned members and schedule appointments.
  • Communicate member progress and care needs to providers and the care team.
  • Explain and translate care information on behalf of members when necessary.
  • Address member questions, process requests, and investigate or direct complaints.
  • Enroll and disenroll members according to established policies.
  • Maintain knowledge of housing, financial, transportation, food, and other community resources.
  • Follow protocols for pain, safety, and mental health concerns and create safety plans when necessary.

Job description

  • Maintain a caseload of up to 50 members and conduct monthly in-person visits

  • Recognize and appropriately report signs of child and elder abuse
  • Follow departmental, companywide, safety, and injury-prevention policies

Requirements
  • Fluent in English, written and verbal
  • Bilingual in Spanish, Russian, Arabic, or Vietnamese
  • Bachelor’s degree in sociology, social work, public health, physiology, or a related field
  • Minimum 1+ years of experience in healthcare, social work, or care coordination
  • Current and valid driver’s license with a clean driving record
  • Reliable transportation and proof of auto insurance
  • Current BLS certification from the American Heart Association upon start date
  • Distraction-free home workspace with a secure internet connection
  • Clear in-person, telephone, and electronic communication ability
  • Adequate hearing and vision, with corrective devices if necessary
  • Problem-solving and logical reasoning ability
  • Excellent organizational skills and attention to detail
  • Commitment to patient confidentiality and healthcare ethics
  • Ability to lift, carry, push, or pull 20–25 pounds
  • Ability to climb stairs and navigate uneven terrain
  • Ability to bend, reach, remain stationary, move around the office, and travel throughout the community
  • Ability to operate a vehicle and travel to member visits, meetings, and events
  • Ability to work independently within established policies and procedures
  • Computer, email, virtual platform, and Microsoft Office proficiency
  • Frequent computer, keyboard, and handheld/mobile device use
  • Ability to type for extended periods
