Job summary

Connecticut Pediatrics @ CHC - Hartford is seeking a psychosocial clinician to meet the mental health needs of patients and families. The role involves clinical assessment, crisis intervention, supportive counseling, case management and collaboration with the integrated health care team on-site in Hartford.

Required: MSW with two years of experience in working with children and families; bilingual Spanish (written and spoken).

Qualifications

• MSW required; LCSW preferred.
• Minimum of two years of related experience with children and families.
• Written and oral bilingual fluency in Spanish required.

Responsibilities

• Assesses patient and family needs and coordinates, advocates and problem solves to enhance functioning.
• Completes psychosocial assessment gathering social, psychosocial, cultural and financial information.
• Develops plan based on assessment and assists family with referrals and follow up.
• Works as part of an interdisciplinary team and accepts warm hand-offs from primary care and others.
• Attends team meetings and supervision as needed.
• Completes required documentation and reports to agencies such as DCF and others.
