Job summary

Legacy Behavioral Health Services in Lowndes County, GA, is seeking a Case Manager to deliver community-based case management and recovery-oriented services. Under supervision, you will provide skills training, education, and progress monitoring for clients and their families, with emphasis on documentation and teamwork.

The role requires a BA in a related field and at least one year of social services experience, plus knowledge of mental illness and a valid Georgia drivers license.

Qualifications

• Bachelor’s degree in a related field and one year of full-time experience in social services delivery.
• Knowledge of mental illness.
• Valid GA driver’s license with less than 5 points.

Responsibilities

• Provide case management and support to clients in the community to promote recovery and independence.
• Monitor client progress toward treatment goals and document services and outcomes.
• Collaborate with community agencies and operate as part of a multidisciplinary team.
• Complete extensive documentation and maintain accurate records of client progress.

Under general supervision performs a wide range of tasks in the community to assist clients in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to clients and their families. Monitor client progress towards treatment goals. Extensive documentation required; accurately charts services and client progress. Must be able to work well with community agencies and function as part of a team..

The Case Manager will be supervised by the Clinical Team Leader.

Requirements:

Completion of a bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field AND one year of full-time experience in social services delivery.

Knowledge of mental illness

Valid GA driver’s license with less than 5 points

Working Conditions: Must be physically and emotionally fit to work in a stress-filled environment to meet deadlines and deal with routine and crisis situations.
