City of Virginia Beach Department of Human Services seeks a CTI Case Manager for the Permanent Supportive Housing program in Behavioral Health. The ideal candidate holds QMHP-Adult licensure, with experience with substance use, serious mental illness, and homelessness in community settings.

The position includes a $4,000 signing bonus and a schedule of Monday–Friday 8:30am–5:00pm with one evening. SOAR certification and EHR experience are preferred.

Qualifications

  • Requires a four-year degree in a human services field and three years' related experience.
  • Must have a valid driver's license.
  • Must be registered as a QMHP with the Virginia Board of Counseling.
  • Experience with substance use, serious mental health disorders, and homelessness in community settings is preferred.
  • SOAR certification is preferred.

Responsibilities

  • Assess client needs and develop care plans.
  • Coordinate services and advocate for housing, entitlements, and medical care.
  • Provide counseling and maintain client records with CTI model.
  • Utilize CTI model to conduct case management and engagement.
  • Complete assessments, crisis planning, and transition planning for PSH program.
  • Work schedule: Mon-Fri 8:30am-5:00pm with one evening.

Hiring bonuses shall only be for new hires of original employment or reemployment as defined in Human Resources Policy No. 5.0.$1,000 will be awarded in the first paycheck.$1,250 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of “Meets Expectations”.$1,750 will be awarded upon completion of the employee’s one (1) year probationary period and a performance evaluation with an overall rating of “Meets Expectations”.The City of Virginia Beach Department of Human Services is seeking to hire a candidate for the Critical Time Intervention (CTI) Case Manager position with the Permanent Supportive Housing program in the Behavioral Health division.The ideal candidate will be registered as a Qualified Mental Health Professional (QMHP) or higher-level license with the Virginia Board of Counseling, has experience working with individuals with substance use and serious mental health disorders and experience working with homeless populations in community-based settings.Duties:Perform clinically professional work assessing client needs.Develop, implement, and monitor service delivery and assistance plans.Act as an advocate and liaison for participants in securing housing, entitlements, treatment, medical care, and other community services and supports.Coordinate and monitor services with other agencies.Provide counseling to clients and assists them.Maintain records and files, prepare reports, and perform other related work as apparent or assigned to clients in a Permanent Supportive Housing (PSH) program.Utilize Critical Time Intervention (CTI) model to achieve.Provide CTI case management services to assigned caseload to include engagement and rapport-building.Refer and connect with appropriate services including long-term mental health and natural supports.Monitor agreed upon support plans, and transitioning of services to long-term services, as needed.Collaborate and coordinate with Permanent Supportive Housing Team including Housing Specialists and Peers on a frequent basis.Complete complex assessments, treatment planning, crisis planning, clinical intervention, consultation, supportive counseling, transition planning and other case management duties for the Permanent Supportive Housing Program utilizing the CTI evidence-based model.Complete clinical documentation and assures all filing of clinical information and submission of billing information are done accurately and within the time frame required.Work Schedule: Monday - Friday 8:30am to 5:00pm (with one evening)Must be registered as a Qualified Mental Health Professional-Adult (QMHP-A) with the Virginia Board of Counseling.Preferences:Six (6) months of case manager experience.One (1) year of experience working with individuals with substance use and serious mental health disorders.Six (6) months of experience working with homeless populations in community-based settings.SOAR certified.Experience with Electronic Health Records.The City offers a generous benefits package which includes health, dental and life insurance, retirement, and savings plans, maternity/paternity and parental leave, Holidays and paid leave.Additional requirements:• DMV Transcript: Yes• CDL: No• Drug & Alcohol Clearinghouse: No• CPS Check: Yes• Physical: No• Respirator: No• Psychological Screening: No• Polygraph: No• At Will: NoMinimum Requirements:Minimum Qualifications: Requires a four (4) year degree in a human services field (such as psychology, behavioral or mental health, special education, social work) and three (3) years’ experience in fields providing the required knowledge, skills, and abilities and associated with such positions as Mental Health Counselor, Special Education Teacher, or Social Worker OR any combination of education (above the high school level) and/or experience and training equivalent to seven (7) years in fields providing the required knowledge, skills, and abilities. Must have a current and valid driver’s license.Special Requirements: Must be registered as a Qualified Mental Health Professional (QMHP) with the Virginia Board of Counseling.
