Job summary

St. Croix Hospice is seeking a Bereavement Coordinator to support its bereavement program for families after a patient’s death. You will provide direct bereavement support, develop care plans, schedule contacts, and reference community services while ensuring HIPAA compliance.

The role emphasizes compassionate counseling and meticulous charting in a hospice setting. Requires a bachelor’s degree in a related field and at least 3 years of bereavement experience, with strong communication skills

Qualifications

• Bachelor’s degree in social work, psychology, counseling, pastoral counseling/chaplaincy, or related field.
• 3+ years’ experience in bereavement services.
• Ability to pass DHS background study.
• Strong communication and interpersonal skills.
• Knowledge of HIPAA regulations and confidential handling of patient information.

Responsibilities

• Supports the growth and development of the bereavement program.
• Develops and implements the bereavement care plan.
• Schedules and completes bereavement contacts and assessments.
• Provides grief counseling and information and referrals to community services.
• Maintains bereavement charts and HIPAA compliance.
