Job summary

St. Croix Hospice is seeking a Social Worker to join a patient-care team delivering compassionate end-of-life support in patients' homes and communities.

You will assess psychosocial needs, coordinate with clinicians, and connect families with community resources to improve quality of life. Qualifications include a CSWE-accredited bachelor's in social work, state licensure, and experience in bereavement or crisis support.

Qualifications

• Bachelor's degree in social work required (CSWE accredited).
• Current license in the state(s) of practice.
• Licensure levels: LBSW, LMSW, LICSW or compact license accepted.

Responsibilities

• Assess psychosocial status of patients and families; coordinate with care team.
• Provide crisis intervention and bereavement support as needed.
• Educate and support patients and families regarding planning and resources.
