Job summary

Primary Health Care, Inc (PHC) is seeking a Behavioral Health Practitioner to work with a multidisciplinary team in a Des Moines primary care setting. You will assess and treat children, adolescents, adults, and families with short and long-term therapy, in clinic or via telehealth.

The role emphasizes collaboration with medical and psychiatry providers, development of treatment plans, and referrals to community resources. Strong Spanish/English skills and pediatric experience are preferred.

Qualifications

• Master’s degree in Social Work, Marital and Family Therapy, Clinical Mental Health Counseling or related field.
• Licensed by Iowa for LISW/LMSW/LMHC/LMFT within the stated timeline.
• Basic Life Support for Healthcare Providers must be obtained within 90 days.

Responsibilities

• Provide behavioral health care per PHC standards and policies.
• Collaborate with PHC providers to ensure comprehensive services.
• Deliver brief and long-term therapy to individuals and families (in clinic or telehealth).
• Develop and update treatment plans and assess health risk behaviors.
• Refer to internal and external resources and coordinate specialty referrals.
• Maintain accurate documentation and manage PHC databases.
• Provide clinical education sessions on behavioral health topics as requested.
• Adhere to regulatory requirements and PHC policies.
