Kids for the Future seeks a Behavioral Health Therapist to provide direct counseling, assessment, and care coordination within an integrated care team. You will conduct comprehensive assessments, develop ISPs, and deliver CPST and TBS services, including group programming and home visits.

Local travel and regulatory compliance are required; licensure in behavioral health and NPI are mandatory, with background checks completed prior to contract start.

Qualifications

• Bachelor’s degree in Human Services or a closely related field required.
• Must be credentialed and in good standing in a behavioral health discipline (e.g., SW-T, MFT, LISW, LPCC).
• Must possess or be eligible to obtain NPI and pass background checks before contract start.

Responsabilités

• Provide direct clinical counseling and care coordination as part of an integrated care team.
• Complete comprehensive clinical assessments and develop individualized service plans (ISPs).
• Deliver CPST and TBS services and conduct group counseling as appropriate.
• Conduct assessments, home visits, and case management within Medicaid/state requirements.
• Coordinate with therapists, case managers, and LTSS partners; ensure documentation compliance.
