Job summary

MyCHN (Community Health Network) in Texas seeks a Behavioral Health Therapist to deliver comprehensive behavioral health services across ages as part of an integrated care team. This nonprofit FQHC serves the Greater Houston area, guided by empathy, transparency, quality, and value.

You will conduct assessments, build care plans, and provide counseling while coordinating referrals and documenting interactions to support whole-person care in the community.

Qualifications

• Active and unrestricted Texas license as an LCSW or LPC.
• Ability to practice independently.
• Eligibility for Medicare/Medicaid provider participation.
• At least one year of clinical experience in area of practice.
• Strong communication and interpersonal skills.
• Ability to work effectively with diverse populations.
• Commitment to patient-centered care and positive outcomes.

Responsibilities

• Conduct behavioral health assessments and develop care plans.
• Gather patient histories through interviews with patients and families.
• Provide individual, couples, family, group, and crisis counseling.
• Coordinate referrals for specialized care when needed.
• Collaborate with providers and interdisciplinary care teams.
• Educate patients on behavioral health conditions and healthy activities.
• Document all patient interactions timely and accurately.
• Manage follow-up care and timely patient communication.
• Support integrated care coordination across behavioral health and primary care.
