MetroWest Medical Center in Framingham, MA, seeks a patient care support professional to assess patients and develop individualized Master Treatment Plans with monthly reviews. You will perform mini-mental status exams, psychosocial assessments, and facilitate both individual and group therapy, documenting interactions per guidelines.

Responsibilities include updating treatment plans every 30 days, contacting absent patients after 3 days, and ensuring compliance with state and federal

Qualifications

• Master's degree in Social Work or related field required.
• Minimum of 2 years of related experience.
• Currently licensed, certified, or registered to practice as required by state law, regulation or policy.
• CPR certification is required.

Responsibilities

• Designs a Master Treatment Plan for each patient based on assessments.
• Reviews and updates treatment plans every 30 days.
• Performs mini-mental status exam and psychosocial assessments on new patients.
• Provides group and individual therapy and timely documentation in line with guidelines.
• Performs related duties as required.
