You will monitor patients, assist with activities of daily living, and help implement treatment plans while collaborating with a multidisciplinary team. This part-time/PRN role offers growth within a mission-driven organization that values client-centered care.

Qualifications

• High School education or equivalent and two years of responsible employment history.
• Experience in Human Services or related field preferred.
• A valid driver's license and a reliable vehicle.
• Be able to obtain a Class E driver’s license if required.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Flexibility and adaptability in a fast-paced environment.

Responsibilities

• Actively monitor assigned patients and attend to immediate needs.

• Lead recreational and group activities to support therapy goals.
• Support discharge planning and coordination.
