Job summary

Avera Behavioral Health Center-Sioux Falls provides physical and psychological care to patients under the direct supervision of a Registered Nurse. The role supports individualized care plans, documentation, and inter-disciplinary collaboration to enhance patient outcomes.

The position involves 1-on-1 or small group care, participation in daily treatment meetings, suicide precautions, and ongoing assessment to guide care decisions while maintaining safety and confidentiality.

Qualifications

• Bachelor's degree in a human services or medical field or related area.
• Associate's Behavioral Health preferred.
• Ability to communicate effectively and maintain confidentiality.

Responsibilities

• Works with individual patients in a 1-on-1 setting or in a group setting aligned with nursing care plans.
• Participates in daily interdisciplinary treatment team meetings and provides input on patient behaviors.
• Implements suicide precautions as needed.
• Assesses changes in patient condition/behavior and notifies RN.
• Supports patient/family programs and maintains safety for all participants.
• Assists with activities of daily living and vital signs collection.
• Contributes to nursing care plan creation and implements therapeutic activities.
• Maintains accurate EMR records and documents promptly.
• Assesses agitation and participates in behavioral management as needed.
• Participates in updating and developing new therapeutic patient groups with oversight.
