Job summary

Sevita's Northwoods Group Home in Glendale, AZ is hiring for a Full-Time caregiver Fri-Mon, 3p-11p. Pay rate is $16 per hour with full compensation and benefits for full-time staff.

Join a mission-driven team providing daily living support to individuals with intellectual and developmental disabilities, assisting with activities, transportation, and community integration while documenting progress and coordinating with care providers.

Qualifications

• High school diploma or equivalent is required.
• Six months of experience in human services preferred.
• Must be 21 years of age.
• Valid driver’s license in good standing.
• Proficient use of computers; ability to obtain a level one fingerprint card.

Responsibilities

• Assist with daily activities including socialization, personal care, housekeeping, and transportation.
• Accurately documents progress and reviews records to stay updated with changes in service plans.
• Provide clinical expertise for specialized populations and behavioral supports.
• Accompany individuals to medical appointments and travel between sites and homes.

Pay Rate $16 Hourly

OUR MISSION AND PERFORMANCE EXPECTATIONS

• Provide assistance with daily activities that range from socialization and behavioral development, personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately documents progress and activity; reviews records and logs to stay up-to-date with changes in service plans.
• Provide clinical expertise which may include knowledge for specialized populations, such as intellectual and developmental disabilities, brain injury, emotional or behavioral disability, or expertise in special disciplines such as behavioral support, early intervention, crisis intervention, or others.
• May accompany individuals we serve to medical appointments; relay orders and information to and from medical providers. Travel between sites, to individual’s homes, and may accompany them into the community.

Qualifications

• High school diploma or equivalent required
