La Clinica in Medford, Oregon is seeking a Behavioral Health Support Specialist to assist patients with behavioral health goals and coordinate care with primary care teams and partners.

The BHSS will screen for health concerns, provide coaching, and refer patients to BHCs for assessment and care planning, while maintaining trauma-informed, empathetic engagement and accurate EPIC documentation.

Qualifications

  • Minimum: High School Diploma.
  • Bilingual required.
  • Strong interpersonal and problem-solving skills.
  • Pursue QMHA certification within 3 years.
  • Preferred: 2–3 years in social services; health coaching or related certification.

Responsibilities

  • Support referral to behavioral health services and coordinate care.
  • Provide follow-up support in person or by phone to patient goals.
  • Identify need for intensive case management and refer to partners.
  • Screen for PHQ, SBIRT, GAD and other health screenings.
  • Coach patients with behavioral health needs.
  • Refer to BHCs for assessment and follow care plans.
  • Screen for suicide/domestic violence; create safety plans.
  • Support group visits and help with curriculum development.
  • Use motivational interviewing to gauge readiness to change.
  • Apply trauma-informed care and document in EPIC.

We’re looking for a Behavioral Health Support Specialist (BHSS) for Birch Grove Health Center, which provides primary care services to people with significant mental health and substance use needs. The BHSS will assist patients in achieving their goals, improving overall well‑being, and coordinate care with primary care team members and community partners.

Responsibilities
  • Support patient and team in identifying and advocating for referral to behavioral health‑specific services (internal or external).
  • Provide ongoing follow‑up support in‑person or by telephone to support patient‑identified goals.
  • Identify when patients may need more intensive case management support and refer to community partners when indicated.
  • Screen and provide interventions for a variety of health screenings (e.g., PHQ, SBIRT, GAD).
  • Provide coaching to patients with behavioral health needs.
  • Refer patients with behavioral health needs to BHCs for assessment and intervention and follow care plans.
  • Screen and intervene for persons at risk for suicide or domestic violence; create safety plans; consult and collaborate with BHCs and PCPs.
  • Assist PCPs and BHCs in delivery of group visits and collaborate on group facilitation and curriculum development.
  • Utilize motivational interviewing to assess patient readiness to change.
  • Employ a trauma‑informed care practice of engagement with patients.
  • Ensure clear, concise documentation in EPIC within scope of practice and organizational policies.
  • Participate in care team huddles, discipline‑specific team meetings, and monthly case consultation meetings.
Required
  • Minimum: High School Diploma
  • Bilingual
  • Excellent interpersonal communication and problem‑solving skills
  • Ability to intervene and promote reconciliation, compromise, and positive outcomes in difficult interactions
  • Working toward or holding a Qualified Mental Health Associate (QMHA) certification within 3 years of employment, maintaining certification while employed
Preferred
  • Two to three years of experience in the social services field
  • Certification in health coaching, maternal and child health, alcohol and drug counseling, or other healthcare‑related certification
  • Experience working with health‑related issues, significant psychosocial needs, substance abuse, and history of trauma
  • Bachelor’s degree in health education, health promotion, psychology, or social work
  • Basic knowledge of adult learning models
  • Proficiency with electronic health records, including EPIC and Wisdom
  • Ability to de‑escalate difficult situations
