Job Description

Brightside and Mercy Medical Center, members of Trinity Health of New England, are seeking an experienced Behavioral Health Specialist / Social Worker to join our team of highly specialized integrated behavioral health professionals.

The Behavioral Health Specialist (BHS), a master's level professional, plays a key role in population health management, including integrating primary and behavioral health care, supporting physicians and the collaborative care team. The BHS works with patients with behavioral health conditions, focusing on social determinants impacting health outcomes. Responsibilities include crisis intervention, connecting patients to social and community services, follow-up, and streamlining services to improve patient experience, health outcomes, and reduce costs.

Responsibilities
  • Build caring relationships with the target population to promote positive health and social service outcomes.
  • Improve access to health and human services through in-person interactions, phone contacts, and referral coordination.
  • Complete assessments and develop strength-based, solution-oriented treatment plans with patients, families, and service providers.
  • Ensure the implementation of treatment plans with all providers.
  • Educate patients on strategies for change, assessing readiness and providing appropriate education.
  • Participate in case conferences, staffing meetings, treatment teams, and other consultations.
  • Demonstrate problem-solving, conflict resolution, and positive communication within teamwork; support the Brightside team as needed.
Minimum Qualifications
  • Master’s degree in social work, marriage and family therapy, psychology, or mental health counseling.
  • Licensed or eligible for licensure preferred.
  • 1-3 years of experience working with at-risk populations, with experience in wrap-around services.
  • Strong communication skills with target populations; culturally competent interpersonal and communication skills.
  • Knowledge of the local community.
