Job summary

Third Street Family Health Services is seeking a Behavioral Health Specialist assigned to Forensic Services to provide community-based behavioral health services, case management, and care coordination to justice-involved individuals and clients in the Forensic Monitoring Program.

You will help clients maintain community stability, access resources, navigate justice system requirements, and address barriers impacting behavioral health and well-being.

Qualifications

• High school diploma or GED required.
• Meets Ohio Medicaid requirements for QMHS.
• Valid driver’s license and reliable transportation required.
• Must obtain BLS certification and complete credentialing within 90 days of hire.

Responsibilities

• Provides community-based behavioral health interventions and supportive services.
• Engages clients and families in recovery-oriented, person-centered care.
• Supports clients in developing coping, independent living, and self-management skills.
• Coordinates care and referrals with internal and external providers.
• Assists clients with accessing community resources and services.
• Recognizes and responds appropriately to behavioral health crises.
• Completes required documentation and maintains compliance with applicable requirements.
• Works under clinical supervision and participates in required training and competency development.
