In the PCBH model, you will coordinate integrated behavioral health services, work with care teams, and focus on improving patient outcomes.

Qualifications

• Graduate degree in mental health field.
• Two years’ experience in individual or community health education.
• Active WA state license as LICSW/LMFT/LMHC; English fluency required.

Responsibilities

• Collaborate with care teams including PCPs, psychiatrists and medical staff.
• Meet performance expectations and assist grant continuation.

Job Summary

ICHS’ behavioral health services are collaborative, patient centered, and designed to improve and enhance patient’s emotional, mental, and behavioral health. As a member of the patient’s care team, ICHS’ behavioral health program is accountable for the behavioral health care of all ICHS patients through direct referral, indirect review and implementation of guidelines and workflows impacted by the patient’s behavioral health. Patients access behavioral health services as a member of their care team. In addition to patient care within the framework of established behavioral health guidelines and ICHS’ Principles of Practice and approved by the appropriate licensing board in the State of Washington, the Behavioral Health Specialist functions as a member of the collaborative care team that includes the patient, primary care provider, behavioral health provider, consulting psychiatrist, and other medical staff. This position is responsible for providing integrated behavioral health care within a primary care setting via the Primary Care-Behavioral Health (PCBH) model of care. This position is subject to meeting performance expectations and grant continuation.

Education

Graduate degree in mental health field (social work, psychology, counseling, or related field).

Experience

Two (2) years’ experience providing individual or community health education.

Other Requirement(s)

Full, active and unrestricted license to practice as a behavioral healthcare provider in WA State (LICSW, LMFT, LMHC). Fluency in English required: additional fluency in other languages is valued.
