Role Description:

The purpose of a Behavioral Health Specialist at Oak Street Health is to provide care management services and evidence-based short-term psychotherapy to patients with behavioral health needs. They are key resources for Care Teams across primary clinics, responsible for coordinating behavioral healthcare, assessing patient needs, making referrals, and providing short-term counseling. This role may offer remote work days based on performance metrics.

Core Responsibilities:

• Collaborate with primary care teams to identify appropriate patients for behavioral health referrals
• Screen and assess patients for mental health and substance use disorders
• Develop care plans and facilitate referrals to psychiatric or external resources
• Provide short-term counseling and evidence-based treatments
• Coordinate care with psychiatric consultants, including telehealth handoffs
• Maintain accurate documentation in electronic medical records
• Participate in regional efforts to improve the Behavioral Health Program
• Potentially cover multiple clinics depending on needs
• Perform other duties as assigned

Qualifications:

Required:

• Master’s Degree in mental health counseling or related field from an accredited institution
• Highest clinical license or local equivalent
• Experience with mental health and substance use disorder screening and assessment
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral Therapy)
• US work authorization

• Proficiency in intervention techniques
• Knowledge of differential diagnoses

Work Hours & Compensation:

40 hours/week, full-time. Pay range: $66,575 - $142,576, dependent on experience, education, and location. Eligible for bonuses and incentives.
