Responsibilities

• Develops psychosocial assessments and treatment plans identifying strengths, needs, and cultural considerations.
• Facilitates psychoeducational and process groups, including Mindability: Roadmap to Peace of Mind.
• Coordinates care with outpatient providers and community resources; participates in discharge planning.
• Maintains timely, accurate documentation in the EMR according to regulations and policy.
• Provides crisis intervention and de-escalation within scope of role and facility training.
