Job summary

Children’s National Hospital in Washington, DC seeks a Behavioral Health Care Manager (BHCM) within the Collaborative Care Model to provide tailored behavioral health treatment, coordination, and brief therapeutic interventions for patients on their panel.

The role requires a Master’s in Social Work, DC LGSW licensure or eligibility, and at least 1 year post-MSW experience in a complex medical setting; strong communication and cultural competency are essential.

Qualifications

• Master's Degree in Social Work is required.
• 1 year post-MSW experience or second year field placement in a complex medical setting.
• Licensed in DC at minimum LGSW level or eligibility with proof of pending DC licensure.

Responsibilities

• Provide comprehensive psychosocial assessment for identified patients and families.
• Develop and implement intervention plans based on assessment.
• Coordinate care and refer to appropriate services.
• Document assessments and interventions in the patient’s medical record.
• Educate staff about psychosocial issues impacting treatment plans.
• Facilitate the discharge planning process with Case Management, nursing, and physicians.
• Advocate for patient and family needs within the hospital system.
