Required is a Master’s in Social Work, 1 year post-MSW experience (or field placement) in a medical setting, and DC licensure or proof of pending licensure. The position emphasizes teamwork with physicians and community resources.

Qualifications

• Master's Degree in Social Work required.
• 1 year post-MSW experience as Social Worker or a second year field placement in a complex medical setting required.
• Licensed in the District of Columbia at minimum at the LGSW level, or licensed in another state with DC licensure proof.

Responsibilities

• Provide comprehensive psychosocial assessment and tailor interventions.
• Coordinate care and refer to appropriate services within the Collaborative Care Model.
• Communicate with enrolled members to assess needs and monitor treatment progress.
• Document assessments and interventions in the medical record per standards.
• Facilitate discharge planning and multidisciplinary communication.
