Job summary

Qualifications

• MSW is required for the role.
• LMSW or LCSW are preferred qualifications.
• Minimum of three years of experience in social work or behavioral health care.

Responsibilities

• Function as the Care Manager within the integrated primary care and behavioral health team.
• Enroll eligible patients into the Circle of Care project and track progress toward outcomes.
• Conduct face-to-face visits and calls with Circle of Care patients during enrollment (3–12 months).
• Establish and maintain a patient registry to monitor clinical outcomes and care processes.
• Coordinate inpatient/partial hospitalization/intensive outpatient programs and discharge coordination.
• Maintain cooperative referral relationships and update referral listings.
