Job summary

Carrollton Springs is seeking an experienced Registered Nurse (RN) for Behavioral Health outpatient services. This PRN days position supports patient assessments, care planning, and collaboration with the interdisciplinary team in a 72-bed facility in Carrollton, TX.

You will perform nursing interventions, ensure documentation accuracy, supervise shift staff, and uphold hospital policies while delivering compassionate, high-quality care to patients and families in a stressful setting.

Qualifications

• Associate's degree in nursing required.
• Current Registered Nurse license as required by state regulations.
• CPR certification required within 30 days of employment.
• De-escalation certification required within 30 days of employment.
• 1 year experience in a psychiatric health care facility preferred.

Responsibilities

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides care appropriate to condition and age of the patient.
• Performs timely and accurate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.
• Oversee and supervise the shift and staff to ensure policy adherence and quality care.
• Monitor patient for changes in condition and respond accordingly, up to notifying the physician.
