MercyOne North Iowa Medical Center in Mason City, IA, is seeking a Behavioral Health Nurse to provide assessments, care coordination and education across the behavioral health continuum within inpatient and outpatient programs. Qualifications

• Current RN Licensure in the state of Iowa.
• Bachelor of Science in Nursing preferred.
• Ability to be non-judgmental regarding living and health related decisions.

Responsibilities

• Assesses and evaluates individuals' physical, psychosocial, environmental, spiritual, social and education needs.
• Works collaboratively with Behavioral Health inpatient and outpatient teams to achieve patient care outcomes and smooth transition of care.
• Provides, facilitates, and/or implements learning opportunities to meet educational needs of patients, families or community members.
• Documents patient information in the medical record in an accurate, timely manner.
• Provides competent patient care to indicated populations by interpreting data and making appropriate decisions based on age-specific growth and development.
• Effectively communicates pertinent patient changes/conditions to colleagues and multidisciplinary caregivers.
