MercyOne Medical Group – North Iowa seeks a Behavioral Health Nurse to provide assessments, support and care coordination across the behavioral health continuum for patients and families seeking treatment options.

The role collaborates with inpatient and outpatient teams, delivers education, and ensures accurate documentation while supporting smooth transitions of care. Night shift, full-time schedule with weekend/holiday rotation.

Formación

• Current RN Licensure in the state of Iowa, or qualified multistate licensure required.
• Bachelor of Science in Nursing preferred
• Ability to be non-judgmental regarding living and health related decisions.

Responsabilidades

• Assesses and evaluates individuals' physical, psychosocial, environmental, spiritual, social and education needs.
• Works collaboratively with the Behavioral Health inpatient and outpatient teams to achieve desired patient care outcomes and ensure a smooth transition of care
• Provides, facilitates, and/or implements learning opportunities to meet the educational needs of patients, families or members of the community.
• When required, documents patient information in the electronic/paper medical record in an accurate, timely manner
• Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.
• Effectively communicates pertinent patient changes/conditions to colleagues and to multidisciplinary caregivers.
