Job summary

Humboldt Park Health is seeking a compassionate Registered Nurse (RN) to join the Behavioral Health team. The RN leads the designated care team on assigned shifts, provides safe, patient-centered care, and coordinates treatment for individuals with behavioral health needs and concurrent medical conditions.

The RN acts as primary care provider for high-risk patients, ensures timely documentation, and collaborates with interdisciplinary teams to optimize care outcomes.

Qualifications

• BLS certification is required.
• CPI certification required within six months of hire.
• National behavioral health or specialty certification preferred.
• Candidates must maintain required licensure and certifications throughout employment.

Responsibilities

• Completes assessments and treatment plans with data from multiple sources.
• Coordinates with psychiatrist and physicians to update treatment plan.
• Develops a nursing plan of care with patient and treatment team.
• Supports patient/family in implementing the plan of care and problem solving.
• Educates patients about disease processes, medications and self-management.
• Completes medication reconciliation and provides information during transitions of care.
• Refers to disease-specific organizations and community services.
• Coordinates longitudinal health and community services.
• Monitors patients regularly and documents progress.
• Reassesses patients each shift and revises the plan accordingly.
• Maintains communication among team members and advocates for patients.
