Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to homebound patients in Clermont, FL. You will coordinate with a interdisciplinary team and referring agencies, ensuring care plans are current and compliant with regulatory standards.

The role requires an RN with 1–2 years in psychiatric settings, valid driver’s license, CPR, and reliable transportation.

Formación

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• 1–2 years of nursing experience (within the last 3 years preferred) in psychiatric care settings.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• Proof of CPR and Hepatitis vaccination or declination.
• Reliable transportation with valid auto liability insurance.

Responsabilidades

• Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and regularly update individualized plans of care.
• Provide skilled nursing interventions related to behavioral health and medical conditions.
• Coordinate care with physicians, interdisciplinary team members, patients and families.
• Ensure patient medication profiles are accurate and up to date.
