Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to homebound patients. The role requires coordinating care with the interdisciplinary team, patients, families, and referring agencies while maintaining clinical standards.
Qualifications include RN licensure, nursing experience in psychiatric settings, CPR, and a valid driver’s license.
Formación
- Graduate of an accredited nursing program (Diploma/AS/BSN).
- Current state RN license and CPR certification required.
- 1-2 years' nursing experience in acute psychiatric settings preferred.
Responsabilidades
- Perform psychiatric in-home assessments and identify clinical needs.
- Develop and update individualized care plans for quality care.
- Provide behavioral health nursing interventions within scope of practice.
- Coordinate with physicians, teams, families, and referral agencies.
- Maintain accurate medication profiles and regulatory documentation.
- Complete admissions, recertifications, and discharges per standards.
- Supervise Home Health Aides and participate in QA activities.
- Utilize EMR systems and respond to on-call rotations.
