Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to homebound patients in Florida. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies, while managing care coordination responsibilities.

The role requires 1–2 years of psychiatric nursing experience, Florida RN license, CPR, and reliable transportation.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• 1-2 years of nursing experience in a psychiatric setting preferred within the last 3 years.
• Current state license as a Registered Nurse (Florida).
• Current state Driver’s License.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial and environmental needs.
• Develop, implement, and update individualized plans of care for quality and continuity of services.
• Provide skilled nursing interventions related to behavioral health and applicable medical conditions.
• Coordinate care with physicians, interdisciplinary team, patients, families, and referral agencies.
• Ensure medication profiles are accurate and up to date.
• Complete admissions, resumptions of care, recertifications, and discharges per regulatory standards.
• Demonstrate knowledge of OASIS and ensure documentation supports medical necessity and homebound status.
• Complete visit documentation promptly, including SNVs and OASIS within 48 hours.
• Use behavioral health and medical assessment tools to evaluate and support patient needs.
• Monitor cases for payer, federal, and state requirements.
• Supervise and provide clinical direction to Home Health Aides as applicable.
• Participate in QA activities and clinical record audits.
• Maintain proficiency in EMR systems and timely syncing of visits.
• Meet productivity standards including visit volume and meetings.
• Attend mandatory meetings, case conferences, and on-call rotation.
• Adhere to HIPAA and professional standards; demonstrate sound clinical judgment.
