Job summary

Ascension St. Francis Hospital in Milwaukee, WI, seeks an experienced registered nurse in Behavioral Health to join our team on days, full-time. You will be responsible for implementing and monitoring patient care plans, coordinating care across disciplines, and recording patient conditions.

You will also ensure orders are executed, participate in discharge planning, and support a compassionate, faith-based environment as part of Ascension's commitment to service and healing.

Qualifications

• RN licensure required prior to hire date.
• BLS Provider certification within 30 days of hire.
• Diploma from an accredited nursing program or equivalent licensure.

Responsibilities

• Implement and monitor patient care plans.
• Monitor, record and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with the healthcare team.
