Blue Ridge Health is seeking a Behavioral Health RN Case Manager to join our ACTT Behavioral Health Team. The role focuses on direct care coordination, patient engagement, and interprofessional collaboration to improve outcomes in a nonprofit FQHC setting in North Carolina.
Candidates should have a BSN, an NC RN license, and at least 1 year in primary care or public health; strong communication and teamwork are essential.
This position is responsible for providing direct care coordination ensuring the care delivered is effective, efficient and achieves desired patient outcomes; This position will work closely with the clinical staff in the clinic to implement coordination of patient care for their behavioral health needs. The Registered Nurse is responsible for providing clinical nursing care to clients according to established professional nursing practice standards of the American Nursing Association and in accordance with the North Carolina Nurse Practice Act and other regulatory standards. This role requires flexibility to transport clients/patients to organization-sponsored services or resources that address their basic and clinical needs. Drivers transporting clients/patients or driving company vehicles are to be age 21 or greater. For those using their personal vehicles to transport clients/patients, a certificate of insurance is required demonstrating at least the minimum required NC auto liability insurance.
