Responsibilities

• Support individuals by providing assistance and hands-on skill building in all areas of daily living including self care and hygiene.
• Laundry and care of clothing, meal preparation, nutrition, dietary needs and menu development.
• Household maintenance and cleaning, budgeting, banking and financial documentation.
• Leisure activities and recreation, awareness and use of community resources.
• Actively participate in residents’ program planning and support independence in goal setting.
• Ensure residents' rights are upheld and personal choices are respected.
• Complete daily shift notes and document progress toward goals; communicate progress to supervisor.

JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member to provide quality services to residents of the BH Adult Residential Program. Individuals in this program need varying levels of support to successfully live in the community and gain skills necessary to move to more independent housing staff in this role play a key role in this goal.

Qualifications

• Minimum requirement of high school diploma or GED. Associates or Bachelors degree in a human services related field plus experience in social services preferred.
• Valid driver's license required.

Specific Duties

• Support individuals by providing assistance and hands-on skill building in all areas of daily living including:
• self care and hygiene
• laundry and care of clothing
• meal preparation, nutrition, dietary needs, menu development, grocery shopping
• household maintenance and cleaning
• budgeting, banking, comparative buying and financial documentation
• leisure activities and recreation
• awareness and use of community resources
• Actively participate in residents’ program planning process and support residents’ independence in choosing goals
• Assure residents rights are upheld and residents’ personal choices are respected
• Ensure daily shift notes are completed and document residents’ progress toward goals within those notes
• Communicate with your supervisor about residents’ progress on goals and provide feedback to individuals regarding their efforts toward their goals
