Position Summary

Under supervision of the Director of Behavioral Health, this role involves working as part of a multidisciplinary primary care team to provide holistic healthcare. Responsibilities include behavioral health and substance abuse screening, brief interventions, education, referrals, and supporting primary care staff with psychosocial, behavioral health, chronic disease, and substance use issues.

Qualifications

• Minimum two years’ experience in a public health setting
• Master’s Degree
• Licensed Clinical Social Worker Certification (LCSW) or Licensed Clinical Mental Health Counselor (LCMHC) in good standing with North Carolina or equivalent

Key Responsibilities

• Provide evidence-based assessments, interventions, consultations, and referrals
• Collaborate with primary healthcare team to improve patient care
• Develop and implement behavioral health, substance abuse, and chronic disease management plans

• Provide consultation and training to healthcare team members
