Overview

We are seeking a Behavioral Health Provider LCSW(A), LCMHC(A), or LMFT(A) to join our team. This position will be based in Charlotte, NC, and Kannapolis, NC, working as part of a multidisciplinary team to provide holistic healthcare for Senior TLC participants in a home and community-based model. Responsibilities include behavioral health and substance abuse screening, counseling, education, and referral services, as well as consultation and treatment planning support for medical staff.

Qualifications

Experience: Minimum two years in a public health setting

Education: Master's Degree

Licensure: Licensed Clinical Social Worker Certification (LCSW or LCSW-A) in good standing with the North Carolina Social Work Certification and Licensure Board (or equivalent)

  • Maintain licensure and professional competence through continuing education
  • Liaise with community agencies and resources
  • Perform other duties as assigned
