The Healthcare Connection in Cincinnati is a National Health Service Corps (NHSC) approved site eligible for application to federal and state loan repayment and scholarship opportunities. We are seeking a Behavioral Health Provider, preferably bilingual Spanish/English, to provide brief, short-term, solution-focused assessments and interventions geared toward functional restoration while consulting with our primary care providers to develop a clear treatment plan for the primary care team to manage.

Who are we?

The mission of The Healthcare Connection is to provide quality, culturally sensitive, and accessible whole person-centered care, including primary health and basic oral services for the underserved, uninsured, and underinsured of Northern Hamilton County. We are the first federally qualified health center (FQHC) in Ohio and among the earliest in the United States. We collaborate with other agencies to meet the complete needs of patients and have received multiple recognitions for our community health efforts.

What is the role of our Behavioral Health Provider?

• Utilize a brief consultation model to communicate with primary care providers, supporting patients and families in self-management, self-efficacy, and behavior change.
• Provide consultation and training to primary care team members in mental health, chemical dependency, and behavioral medicine.
• Manage psychosocial aspects of diseases, develop clinical pathways, and facilitate referrals for additional services.
• Offer behavioral health services and substance abuse screening to diverse patient groups, developing harm reduction strategies.
• Perform comprehensive bio-psychosocial assessments and follow-up, including medication adherence and disease self-management counseling.
• Document all interactions using electronic medical records and electronic tracking tools.
• Assist staff during crises and psychiatric emergencies as needed.

Requirements

• Master’s degree in social work, counseling, or psychology (copy of degree required).
• Licensed in Ohio; supervision available for independent licensure (license copy required).
• At least three years of relevant experience with diverse populations.
• Bilingual Spanish/English preferred.
