Job summary

LaSante Health Center is seeking a compassionate Behavioral Health Case Manager to support patients in our Brooklyn outpatient clinic. You will help patients navigate care, address barriers to treatment, and coordinate services to maintain engagement in behavioral health treatment.

The ideal candidate is personable, proactive, and comfortable serving underserved populations in a fast-paced setting. Strong communication and coordination with providers are essential.

Qualifications

• Minimum high school diploma required; additional degree preferred.
• Strong communication and organizational skills are essential.
• Bilingual candidates encouraged to apply; experience with underserved populations is a plus.
• Experience in healthcare, behavioral health, or care coordination is highly preferred.

Responsibilities

• Assess patients for social, behavioral health, and practical support needs.
• Assist patients with accessing community resources and services.
• Coordinate care between providers and outside organizations; document in EMR.
• Conduct outreach calls to schedule, confirm, and follow up on appointments.
• Support patient engagement and retention in treatment.
• Collaborate with therapists, psychiatrists, medical staff, and admin team.

LaSante Health Center is seeking a compassionate and organized Behavioral Health Case Manager to support patients receiving mental health services within our outpatient behavioral health clinic in Brooklyn.

This role focuses on helping patients navigate care, addressing barriers to treatment, assisting with coordination of services, and maintaining engagement in behavioral health treatment. The ideal candidate is personable, proactive, and comfortable working with underserved populations in a fast-paced clinic environment.

Responsibilities

• Assess patients for social, behavioral health, and practical support needs
• Assist patients with accessing community resources and services
• Help coordinate care between providers and outside organizations
• Conduct outreach calls to schedule, confirm, and follow up on appointments
• Support patient engagement and retention in treatment
• Document interactions and patient needs within the EMR
• Work collaboratively with therapists, psychiatrists, medical providers, and administrative staff
• Assist with referrals, transportation coordination, insurance issues, and other patient support needs

Qualifications

• High school diploma or equivalent required
• Associate’s or bachelor’s degree preferred but not required
• Experience in healthcare, behavioral health, community outreach, care coordination, or customer service strongly preferred
• Strong communication and organizational skills
• Comfortable speaking with patients over the phone and in person
• Bilingual candidates encouraged to apply
• Experience working with Medicaid or underserved populations is a plus
