Qualifications

• Bachelor’s degree from an accredited college or university in Social Work or other related human service field.

Responsibilities

• Interviews patients and families coordinating plans, programs, and activities that meet emotional and social needs.
• Provides crisis intervention, assisting patients in understanding implications and complexities of life situations, relationships, and its impacts on lifestyle.
• Serves as a liaison between the hospital, governmental, and external supports.
• Performs other duties as assigned/required by this position.
