Job summary

Do you have a passion for our community and the skill set to support growth at an up-and-coming health organization? Progressive Community Health Centers is seeking an experienced professional to serve as a Behavioral Health Navigator at our Lisbon Avenue Health Centers.

Does not require working late nights, no holidays, and no weekends!

Who are we?

Founded in 1999, Progressive Community Health Centers is an independent non-profit organization that operates four clinic locations in Milwaukee, Wisconsin. More than 100 employees support our agency's mission to improve the health and quality of life of the community by providing culturally competent services that address identified needs. We provide high-quality, comprehensive care to roughly 16,000 patients annually majority of who are low-income. No one is ever turned away from our clinics based on insurance status or ability to pay. We value a patient-centered philosophy and provide a range of services that include family medicine, internal medicine, women's health, urgent care, dental, radiology and integrated behavioral health. We also offer supportive services such as case management, health education, and benefits enrollment.

What will you do?

The Behavioral Health Navigator involves implementing care plans designed by social workers to address patients' identified needs, resources, and goals. Key responsibilities include educating patients and their families about available options, prioritizing cases based on urgency, and effectively utilizing community resources. The position promotes patient self-management and independence by assisting with essential services such as transportation and housing.

Additionally, the role requires maintaining accurate documentation, participating in departmental meetings, and fostering collaborative teamwork while adhering to organizational policies and ethical standards. The candidate must possess strong communication skills, cultural competency, and the ability to work independently and as part of a team. Knowledge of federal and state regulations, particularly regarding patient confidentiality and HIPAA, is essential.

Schedule

Schedule: Monday - Friday, 8 a.m. - 5 p.m.

Responsibilities
Service - Providing coordinated, patient-centered support that ensures timely, effective, and seamless delivery of care.
  1. Identifies and effectively utilizes community resources to meet the needs of patients/families.
  2. Participates in departmental and organizational committees as applicable.
  3. Promotes collaborative teamwork
Empowerment - Equipping patients and team members with the knowledge, resources, and guidance needed to confidently participate in care and decision-making.
  1. Implements the plan developed by the Social Worker that addresses the patient's identified needs, issues, resources, and care goals; determining the choices available to individual patients; and educating the patient/family on available options; prioritizing patients according to intensity, need, and required follow-up.
  2. Promotes patient self-management and empowers patients and families to achieve maximum wellness and independence. Assists patients with needed items, including transportation, scheduling, housing, food sources, etc.
  3. Maintains professional growth and development.
Compassion - Showing empathy, respect, and understanding in every interaction to support the emotional and physical needs of patients and families.
  1. Maintains appropriate professional boundaries.
  2. Interacts in a manner that is professional, respectful, positive, helpful, and promotes trust.
  3. Acts in a manner consistent with the Code of Conduct.
Quality - Consistently upholding evidence-based standards, safety practices, and continuous improvement to achieve optimal patient outcomes.
  1. Maintains accurate and timely documentation.
  2. Adheres to organizational policies and procedures. Provides input into work processes and policy revisions.
  3. Maintains a working knowledge of and adheres to applicable federal/state regulations, including, but not limited to, laws related to patient confidentiality, release of information, and HIPAA.
EDUCATION AND EXPERIENCE:
Required Qualifications:
  1. A bachelor's degree in social work, human services, psychology, public health, criminal justice, sociology, anthropology, or related field.
Related Work Experience
  1. Experience preferred within Milwaukee County community resource settings.
