Qualifications

Responsibilities

• Manage intake inquiries and assess admission appropriateness and level of care.
• Coordinate with clinical staff for case assignment and treatment team meetings.
• Assist with obtaining specialty supplies for patients as needed.
• Handle crisis calls and connect patients to emergency services when required.
• Serve as liaison between patients, providers and referral sources; ensure HIPAA compliance.
• Document thoroughly in the electronic health record and related systems.
• Support transitions and navigation across the behavioral health continuum.
