Job summary

Southwest Human Development in Phoenix is seeking a Behavioral Health Home Visitor to empower families within the Direct Support Services Program. Through weekly visits and telehealth, you will help build family stability and improve children's behavioral health outcomes, collaborating with the family’s behavioral health home and the Child and Family Team (CFT).

You'll provide parent coaching, education, skills training, and case management, complete intakes and assessments, and connect families

Qualifications

  • 1+ year in behavioral health, social work, or case management.
  • Strong interpersonal and communication skills.
  • Excellent organization for multiple priorities.
  • Ability to navigate Electronic Health Record (EHR) systems.
  • Experience with diverse populations.
  • Effective written and verbal communication.
  • Experience facilitating parent education.
  • Insured vehicle and AZ driver’s license.
  • Valid AZ Fingerprint Clearance Card or eligibility.
  • DES and AZ DHS Criminal Affidavits.

Responsibilities

Job title: Behavioral Health Home Visitor

As a Behavioral Health Home Visitor in the Direct Support Services Program, you’ll make a lasting impact by empowering families and strengthening parenting skills. Through weekly visits, you’ll help build family stability and improve behavioral health outcomes for children. Working closely with the family’s behavioral health home and Child and Family Team (CFT), you’ll provide parent coaching, parenting education, skills training, and case management. You’ll also complete intakes, assessments, individualized treatment plans, and connect families to vital community resources. This role is all about collaboration, compassion, and creating brighter futures for children and families.

What You’ll Do:
  • Conduct weekly home visits and telehealth sessions to strengthen parenting skills, enhance family stability, and improve children’s behavioral health outcomes.

  • AZ DHS Criminal History Affidavit Required
What Sets You Apart:
  • Home visiting experience highly preferred.
  • Experience facilitating parent education and training in a group or 1:1 setting is a plus.
  • Experience writing Individualized Treatment Plans (ITPs) or Individualized Service Plans (ISPs) is a plus.
